This fossil appears to be a
Platyceras snail of a yet undetermined species. It was found in 2002 in a quarry in Clark County, Indiana USA. The layer it was found is called the Jeffersonville Limestone which dates to the Middle Devonian Period. It was part of the James Conkin collection. Thanks to Kenny for updating me about its species name.

It appears to me that it was hooked to something due to the slots in the shell, maybe it was attached to the anal tube of a large crinoid with spines and lived there hooked on. The slots seem to be narrowed around something hard. Just an observation.kenny
